She'll call, text, or email — whichever you chose. The conversation is a real intro: a chance to ask questions, share what's bringing you in, and check fit before anything is scheduled.
She confirms a time and sends a secure deposit link.
If you both decide to move forward, you'll get a one-time link (valid 48 hours) to pay a 50% deposit through Stripe. This is what holds your appointment.
You receive a confirmation and calendar invite.
A confirmation email lands in your inbox with a calendar attachment. Lizanne sends you the Doxy room link or location details personally, separate from this confirmation.
After the session, the balance is settled automatically.
Once Lizanne marks the session complete, the remaining 50% is charged to the same card you used for the deposit. You'll receive a receipt by email.

Lizanne is a Licensed Professional Counselor in Michigan (LPC) and is bound by both state licensing rules and the federal HIPAA Privacy & Security Rules. The way her practice is set up keeps your information safe at every step — here's what that actually means in plain language.
This form & website
Only your name, email, phone, contact preference, and preferred times are collected — by design. The website does not store medical history, diagnosis, or treatment notes. There is no place on this form for clinical information, and there never will be.
Your intro call
When Lizanne calls, the conversation is covered by the same federal privacy rules that protect your record at any clinic. Anything you share is private to your therapeutic relationship with her.
Telehealth sessions
Sessions happen on Doxy.me — a telehealth platform built specifically for HIPAA-covered care, with end-to-end encryption and no recording stored. Lizanne sends you the Doxy room link personally once your appointment is confirmed.
Your clinical record
Once you become a client, your clinical record is kept by Lizanne under Michigan licensing rules and the HIPAA Privacy Rule. You'll receive her Notice of Privacy Practices at intake, and you have the right to request a copy of your record at any time.
Email confirmations
Booking and payment emails route through Zoho's ZeptoMail, a HIPAA-eligible service Lizanne has a Business Associate Agreement (BAA) with. Lizanne never includes diagnosis, treatment specifics, or clinical detail in any email.
Payment processing
Stripe handles your card. They see only your name, email, phone, and a generic line item — "Counseling session — Lizanne Chisholm, LPC." They never see why you're seeing Lizanne or anything about your treatment. Your card details are stored by Stripe (a PCI-DSS Level 1 processor), not on this site.

- When am I actually charged?
- Only after Lizanne has confirmed a time with you and you've decided to move forward. The 50% deposit holds the appointment; the remaining 50% is charged after the session is complete.
- What if my preferred time is taken before Lizanne reviews it?
- It happens. That's why you can pick up to three. If all of yours fill, Lizanne will reach out with the next closest options.
- Can I cancel or reschedule?
- Yes. Just reply to your confirmation email or call. Cancellation policy details are in your client services agreement at intake.
- Is the form HIPAA-protected?
- Yes — and so is the rest of your care. This form deliberately collects no clinical information, and the website does not store medical history, diagnosis, or treatment notes.
